This model was made by me from scratch, including the texture and bump map. It is UV unwrapped and the mesh is mirrored so tweaking the model is very easy. The base mesh is also parented to a null to make moving and rotating the model as easy as possible.
If you look at the UV map, you can see the geometry that I started with. I folded a real piece of paper and measured the angles, then subdivided the mesh to add about five vertices to each fold area. Then, I created a custom transform orientation to rotate around that edge. I manually moved the additional vertices because I couldn't get proportional editing to move them the way I wanted.
It's probably not the best method, but it worked. I also tried using an armature to move the verts, but that didn't work well.

@jefftml, please could you tell me what method you used to make the folds? I have a project that is somewhat similar, but with many more folds, both valley and mountain folds.
I like the fact that you have used paper torn out of a lined paper pad.
